Payment is expected at the time of service. We accept most forms of payment including major credit cards. Package plans are available.

Insurance Acceptance

Integrative Hospital Associates accepts many insurance plans as full or partial payment for professional services. Co-pays and deductibles may apply, and plan benefits will need to be verified by our practice manager. We require that you fax a copy of the front and back of your insurance identification card, your date of birth, and your contact phone number. Please fax this information to 954-473-5993 attention Patsy. We will call you back to discuss your benefits and payment options.

Directions To Our Fort Lauderdale Office  

Our office is located at 4711-A North Dixie Hwy. in Fort Lauderdale. The location is approximately two blocks South of Commercial Blvd. on the West side of North Dixie Highway (in the House of Ladders Plaza).

Directions To Our Davie Office

Our office is located at 2215 S. University Drive in Davie. This is in the Promenade West Shopping Center which is approximately 1/4 mile South of I595 on the West side of University Drive. Directions To Our Coral Springs Office  

Our office is located at 10251 West Sample Road between N. University Drive and Coral Springs Drive. Professional services provided at this location are provided by Dr. Scott Denny and Dr. David B. Bass.

Cancellations are requested 24 hours prior whenever possible. For changes to appointments for Fort Lauderdale or Coral Springs offices please call 888-840-4325. For changes to appointments for the Davie office please call 954-473-8925.

Preparing For Your First Visit

Treatment Expectations

If you have several health conditions, please list them in the order of importance. Please be realistic concerning treatment. It make take several treatment sessions to feel a change. Please create a treatment result diary to note daily changes in how you feel.
