Payment is expected at the time of service. We accept most forms of payment including major credit cards. Package plans are available.

Insurance Acceptance

Integrative Hospital Associates accepts many insurance plans as full or partial payment for professional services. Co-pays and deductibles may apply, and plan benefits will need to be verified by our practice manager. We require that you fax a copy of the front and back of your insurance identification card, your date of birth, and your contact phone number. Please fax this information to 954-473-5993 attention Patsy if you are verifying insurance coverage for the Davie office. If you are verifying insurance coverage for the Fort Lauderdale office please fax the information noted above to 954-537-5522 attention Shasha. We will call you back to discuss your benefits and payment options.

Our office is located at 2020 NE 48th Ct.. in Fort Lauderdale. The location is approximately 1/2 block South of Commercial Blvd. off of NE 20th Ave. 

Directions To Our Davie Office

Our office is located at 2215 S. University Drive in Davie. This is in the Promenade West Shopping Center which is approximately 1/4 mile South of I595 on the West side of University Drive. Directions To Our Boca Raton Office 

Our office is located at 901 Meadows Road, Suite A in Boca Raton. The office is just West of Boca Community Hospital and East of I95.

For Patients Traveling From Other States Or Internationally

There are many convenient hotels in the greater Fort Lauderdale area. Please click here for a list of local hotels. Traveling to and from either the Davie or Fort Lauderdale offices will be a 20 car ride and still be convenient to the airport.

Appointment Cancellations

Cancellations are requested 24 hours prior whenever possible. For changes to appointments for Fort Lauderdale or Coral Springs offices please call 754-200-1555. For changes to appointments for the Davie office please call 954-473-8925.

Preparing For Your First Visit

After finding out which forms you should be filling out, please print out the patient forms and take your time filling them out. Be as descriptive and detailed as possible. Again, please call our office at 743-200-1555 prior to filling out any forms.

Treatment Expectations

If you have several health conditions, please list them in the order of importance. Please be realistic concerning treatment. It make take several treatment sessions to feel a change. Please create a treatment result diary to note daily changes in how you feel.
